Have you ever had a temporary address recorded on your NHS Record?

SWFT Patients: If you have ever had a temporary address recorded on your NHS Record, it might still be listed. This includes addresses used for holidays that were recorded during visits to a GP or hospital.

Please check your records and inform us of any changes by emailing medical.records@swft.nhs.uk. Do include your NHS number and the date you stopped living at the temporary address.

We have partnered with You.Smart.Thing, a travel assistant which helps visitors find the best routes and receive personalised travel updates and live travel news when travelling to Leamington, Stratford and Warwick Hospitals. It’s free to use, there’s nothing to download and you can quickly and easily plan your trip.
