The Diabetes and Tissue Viability Team, work in the community and as part of the Multi-disciplinary Diabetes Footcare Team (MDFT.) The service supports people diagnosed with Diabetes and/or Peripheral Arterial Disease (PAD). People with Diabetes who have active, high or moderate foot risk according to NICE Guideline 19 (2015) and patients with PAD are at risk of developing severe foot complications.
Patients with neuropathy and/or impaired circulation or one or more risk factors, should be referred to the Podiatry Department for triage, screening, advice and care plan. Active foot problems including ulceration with or without infection or suspected Charcot arthropathy, should be referred immediately for urgent triage, assessment and treatment.
