Heart failure nurse service
What we do
We deliver expert nursing care tailored to advise, educate and support heart failure patients and their carers, encouraging them to become active participators in their heart failure management. We are a community based service offering clinics in many locations across South Warwickshire.
We also initiate medications in line with current guidelines. We work alongside the Cardiology consultants and GP colleagues.
