What we do
We support the care of children and young people who have complex and chronic health needs in the community. We provide individual care plans designed specifically around the needs of each child or young person and their family.
By working in partnership with parents and carers and as part of a multi-disciplinary team, we aim to prevent hospital admission, facilitate early discharge and provide excellent care for children with complex needs.

We act as family advocate and liaise with other services on behalf of the child/young person and their family.
