South Warwickshire University NHS Foundation Trust and Warwickshire County Council are working together to commission a number of hospital step down services to help people across Warwickshire get care they need outside an acute setting.
Thanks to additional winter funding the two organisations have been able to introduce three new services that will support people who do not medically need to be in hospital, get the care they need in a more appropriate setting.
The aim is to provide customers with support in their own home for a period of 6 – 12 weeks pending assessment. It is expected that these customers will require the support of 2 carers (double ups) due to their restricted movement and safe moving and handling.
PLEASE NOTE: This pathway is underdevelopment.
A care homes based ‘Reduced Mobility’ step down pathway for individuals deemed as unable to return to their original place of residence due to temporary non weight bearing ability caused by unstable fractures or plaster of paris on their lower limb.
