Specialist Palliative Care Community Service
What we do
We are a specialist service who support those individuals and carers having difficulty with symptom control and emotional issues surrounding their illness and related treatments, particularly when they are coming to the end of their lives.
The role includes:
- Assessment and care planning for patients with complex palliative care needs
- Psychological support for the patient and their families or carers
- Information on disease process, treatment, medication, local and national services
- Advice on symptom control
The service is county-wide and has teams covering North Warwickshire, Rugby and South Warwickshire.
The Specialist Palliative Care Team:
7 days per week between 08.30am to 16.30pm

The role includes:
- Advice on symptom control
- Psychological support for the patient and their families or carers
- Information on disease process, treatment, medication, local and national services
- Assessment of complex palliative care needs
