Did you know South Warwickshire University NHS Foundation Trust has a team that helps support Homelessness in our community?

The Street Outreach Team, formed in January 2020, provides drop-in health assessments, diagnosis, treatments (including prescribing) and evaluation for rough sleepers in North and South Warwickshire.

The team also holds drop-in clinics across our communities, which offer dental, podiatry, sexual health, hepatitis C (HVC) screening, fibro scanning, vaccinations, regular ulcer and wound care at these clinics.

These drop-in clinics are currently in:

  • Nuneaton (The Salvation Army)
  • Rugby (Hope 4)
  • Leamington Spa (Helping Hands Charity)
  • Stratford-upon-Avon (The Fred Winter Centre)

Our Street Outreach Team also work with those who are vulnerably housed, those threatened with eviction and the hidden homeless, like those who sofa surf.

South Warwickshire University NHS Foundation Trust is proud to support the Armed Forces community. We are veteran-aware and reservist-friendly organisation, accredited by the Veterans Covenant Healthcare Alliance.

We primarily support rough sleepers with their medical needs, we will also assist with approaching the councils for homeless applications.

We can also:

  • Help with Personal Independence Payment (PIP) forms
  • Write fit notes
  • Arrange transport for hospital appointments
  • Accompany you to any appointments
  • Make appointments (for example, with the optician, dental, get clothing, food, food vouchers)
  • Look after pets during appointments
  • Accompany to any visits
  • Assist getting benefits
  • Signposting to relevant charities
  • Support getting you to probation appointments
  • Refer to domestic abuse agencies
  • We can refer to the National Referral Mechanism (NRM) for modern slavery
  • Liaise with social workers (especially facilitating working with social services)
  • We have a trusted relationship to enable some meetings (especially with newborns/children)

The team ordinarily work 9am – 5pm, Monday to Friday.

The Integrated Single Point of Access (ISPA) takes all referrals and calls into the service from health professionals, patients and families.
